Add forms and quizzes


Make your designs interactive with forms, polls, or quizzes. Collect feedback, run surveys, test knowledge, or engage your audience with up to 10 questions per form. You can also automatically generate quizzes based on your design's content using AI.

Add a form

Canva forms are available as preset designs, making it easy to quickly add structured, ready-made forms to your design without starting from scratch. These presets help you save time and ensure your form includes common fields and layouts that are easy for your audience to use.

  1. From the editor side panel, select
    Elements.
  2. Under categories, select Forms.
  3. Scroll through the available form types.
  4. To view the forms for each type, select See all.
  5. Select a form to add it to your design.

Generate via Canva AI

  1. From the editor side panel, select
    Elements.
  2. In the Prompt box, describe the form you want to create (for example, a post-event poll or a quiz on land animals). Learn more about AI prompting here.
  3. Select the Generate
    dropdown, then choose Forms. Canva AI will create a preset form based on your prompt.
  4. Review the generated form. If all looks good, select the + plus button (Add to design).

Generate a quiz from your content

Create quizzes automatically based on text in your design.

The Generate quiz feature is available to all users (except in China). It’s powered by Magic Write and included in all Canva plans.

  1. In your design, select 2–3 full sentences of text to base your quiz on. You can also select sticky notes, shapes, or lines with text.
  2. Hold Ctrl (Windows) or Cmd (Mac) to select multiple elements.
  3. In the floating toolbar, select
    Magic Write.
  4. Select Generate quiz.

Tip: If there's not enough content to generate a quiz, select more text and try again. Failed attempts won't affect your AI usage.

Customize your Form and Quiz

Customize questions

  1. Select your form, then choose Edit from the quick-access menu.
  2. In the Questions tab:
    • Add or remove questions (up to 10)
    • Change question types
    • Set required fields
    • Add helper text
    • Mark correct answers for quizzes
  3. In the Settings tab:
  • Toggle to show or hide the header field.
  • Customize the submit button text.
  • Choose whether to collect respondent names.

Customise the appearance of your poll or quiz

  1. Select the poll or quiz you’d like to customize. A quick-access menu with various customization options will open.
  2. Select the Color options to change the colors of your poll or quiz.
  3. Select
    Border style to customize your poll or quiz’s border.
  4. Select Corner rounding to customize the corners of your poll or quiz.
  5. Select
    Transparency to adjust the opacity of your poll or quiz.
  6. Select
    Animate to add an animation to your poll or quiz.
  7. Select Position to adjust the position and alignment of your poll or quiz in your design.
  8. To customize text in your poll or quiz:
    • Select the form you'd like to customize.
    • Use the text editing toolbar to change the font family, adjust the text size, change the text color, or apply other text formatting options.

View the responses of your Form and Quiz

View the results of your poll or quiz

  1. Select the poll or quiz you’d like to check. A quick-access menu with various customization options will open.
  2. Select Responses. A panel that shows the responses should appear.
  3. Select
    More, then
    Download as CSV to download the response data.

Note that if you make changes to your options after your poll/quiz has already collected entries, the data may reflect differently:

  • Deleting an option: The submissions will be labelled as “Inactive” since the deleted option will no longer be available in the poll or quiz.
  • Changing the correct answer in a quiz: The status of previous submissions will be altered to align with the new correct answer. For example, if you initially marked option A as correct and received submissions for option B, those would be marked as incorrect. However, if you later change the correct answer to option B, all prior submissions will now be considered correct.
  • Editing option labels: When you edit an option's label, all submissions will reflect this updated label. If the option is deleted, the label reverts to the one that was in place at the time of submission.

What are the limitations of Polls and Quizzes?

  • You can add up to 10 questions per form.
  • You can add up to 10 options per question.
  • You can insert 10 forms per page, and up to 20 forms per design.
  • Polls and quizzes can be added to all doctypes except Docs and Email.
  • Forms and quizzes must be shared via a Canva link to collect responses.
  • Response anonymity depends on the form’s settings. If name collection is off, responses are anonymous, even from logged-in users. If it’s on, names are collected from logged-in users and may be asked from others. On Canva Websites, responses are always anonymous. Names can’t be collected.
  • Forms won’t collect responses if downloaded as images, videos, or PDFs.
